Idiopathic scoliosis : a spine 3 D deformation but not only...
Skeletal deformation above and below.

The growth program
The growth is very interrelated with the child scoliosis progression.
Spine is divided in 13 territories under the control of different homeobox gene.
Spatio-temporal program of formation, growth and maturation are under the control of pineal gland (biological clock).

In white low velocity of growth, in yellow moderate one, in green high velocity of growth.
Muscular imbalance : local, segmental second and general later on.
Perturbations of spine movements
Compare to the control subject (pictures a and b) demonstrating: a perturbation of the amplitude, a repartition of the movement along the spine during bending, torsion and flexion for patient with right thoracic curve (picture c).
Bone deformations : in frontal, sagittal and horizontal plane.
Risks of aggravation : Predictive criteria
The risk factors of aggravation that we can observe are many, but not always with scientific proof, even if the experience of the professional can identify them perfectly.
The risk factors have specific diversity in relation with one unique person.
All elements form the story of a patient, his clinical exam, his Xrays and complementary exam(s) have to be studied seriously to determine a well adapted treatment option..

Predictive criteria : well known
Predictive criteria : less known
In fact the clinical abnormalities (3D scapula disorientation, hypokyphosis, postural imbalance...) appears in advance, sometimes a long time before the aggravation we can observe on Xrays, worsening will appear after a short period of high velocity of growth for the part of the spine with the primary curve.
The first curve appears is not always the primary one.
Just one curve is very exceptional or not idiopathic.
